With over 10 years of experience in the Western Australian disability and healthcare industry, BrightWest Case Management specializes in supporting individuals living with various disabilities or those who have experienced catastrophic injuries.
We have extensive expertise in assisting participants to navigate and connect with supports funded by various systems. We also support those transitioning from hospitals and other institutions back into the community. Our services are available to participants funded under the National Disability Insurance Scheme (NDIS), Insurance Commission of Western Australia (ICWA) and the Department of Veterans' Affairs (DVA).

Specialist Support Coordination

Support Coordination
Support Coordination is designed to help you navigate and understand the supports in your plan. Our goal is to assist you in maximizing your NDIS plan and connecting you with the service providers necessary to reach your goals. We also provide capacity building supports that enable you to build your confidence to manage your plan.
Psychosocial Recovery Coaching
Our recovery coaches support participants through their recovery journey by building capacity and resilience. We help individuals identify, plan, organize, and maintain supports while developing the strength, knowledge, and skills necessary to navigate daily life.
Case Management
We provide support following complex injuries to help you access the services, treatments, and resources necessary for a smooth transition from hospital to your home or alternative accommodation. Our assistance is available throughout both the rehabilitation phase and beyond.
